Submit the following credentials: A. Freshmen Original copy of NSO birth certificate Certificate of Good Moral Character Original Form 138 (High School Report Card) Four (4) copies of a 2x2 recent colored I.D. picture Medical Clearance (Hepatitis test, chest x-ray, CBC, Urinalysis) B. Transferees Certificate of Transfer Credential Transcript of Records (TOR)/Copy of Grades Certificate of Good Moral Character from school last attended Four (4) copies of a 2x2 recent colored I.D. picture Medical Clearance (Hepatitis test, chest x-ray, CBC, Urinalysis) C. Foreign Students Photocopy of ACR I-card (bring original for verification) Photocopy of Passport (bring original for verification) Original copy of TOR, with English translation and duly authenticated Original and photocopy of Student Visa, Study Permit and valid Passport (if ACR I-card is not available) Four (4) copies of a 2x2 recent colored I.D. picture Medical Clearance (Hepatitis test, chest x-ray, CBC, Urinalysis)
